豁免法律責任及聲明 Waiver of Liability and Declaration*

謹此聲明本人參與「以愛之名」靈實慈善行 2023，並願意遵守由基督教靈實協會、所有贊助商及支持是次活動之團體〔以下簡稱「大會」〕所訂的規則，當中包括：I.本人在登記表格及相關文件提供的資料均正確無誤。II.本人已進行足夠訓練，且身體狀態良好，具備可以完成是項活動之條件，並可自行承擔所有責任。III.本人自願參加是次活動，並已考慮及明白相關的風險及責任，亦會自行購買相關的個人意外保險。IV.本人同意授權大會及傳媒使用相關的活動相片、錄影片段、錄音片段或任何資料及記錄，作為活動推廣及宣傳之用。V. 本人同意接受大會有關活動的宣傳資料及訊息。 I hereby declare participation of the “In the Name of Love” HOHCS Charity Walk 2023, and willing to comply with the regulations stipulated by the HOHCS, all sponsors and supporting groups of this event (refer to as “The organizer” below), including: I. The information that I provide on the registration form and the relevant documents are accurate and correct. II. I have undergone enough training and in good physical condition with the ability to complete the event, and will solely undertake all the responsibilities. III. I volunteer to participate in this event, and have duly considered and understood the relating risks and responsibilities. I will purchase personal accident insurance plan for the event. IV. I agree to authorise the organizer and media use of the relevant event photos, video clips, audio recordings or other information and records for promotion and publicity. V. I agree to receive publicity materials and information from the organizer.

基督教靈實協會 (下稱靈實) 尊重閣下所提交的個人資料，您所提供的個人資料只限用於靈實發出捐款收據、通訊、籌募經費及收集捐款者意見之用。在未得您的事先同意前，我們不會向其他人士或機構披露所收集的個人資料。靈實並會確保所有個人資料都根據《個人資料(私隱)條例》所載的規定妥善保存及使用。如閣下需要查閱或修正你的個人資料，請在辦公時間內致電2703 3290，電郵至 [email protected] 或致函至香港九龍將軍澳靈實路7號與本會傳訊及籌募部聯絡。 The Haven of Hope Christian Service (hereafter as “HOHCS”) respects the personal information that you provide. Your personal information will be solely used for the purposes of sending donation receipts by HOHCS, communication, fundraising appeals and collection of donors’ opinions. We will not disclose your personal information to any third parties or organizations without your prior consent. HOHCS ascertains the preservation and usage of all the personal information in strict adherence to the relevant provisions of the Personal Data (Privacy) Ordinance. 本人同意基督教靈實協會及其服務單位使用本人之個人資料傳遞服務通訊、活動宣傳、發出捐款收據及慈善籌款募捐之用。I agreed that HOHCS and its service units use my personal information for communication, publicity of activities, sending donation receipts and fundraising purposes.

活動條款及細則 Terms & Conditions*

1.大會將於活動當日進行拍攝，用作推廣義務工作。2.大會已購買第三者保險，建議參與參加者如有需要，可自行購買個人及其他有關保險。3.大會建議參加者在登記前及參加活動前先行諮詢醫生意見。4.大會保留本活動之最終決定權，任何有關活動的臨時活動改動或消息，將以「基督教靈實協會」Facebook專頁及官方網頁公佈為準。5.所有獎項均根據大會記錄為準。大會不設上訴機制，並保留最終決定權。6.所有獎項只會於指定時間頒發，不設補領、預領和代領。7.登記表格一經遞交，即代表相關人士確認及同意接受一切活動的規則、條款和安排。8.大會擁有修改及解釋以上規則的權利，而不作另行通知。如有任何爭議，大會保留就爭議及與此活動有關任何問題之最終決定權。 1.The organizer will conduct shooting on the event day for promotion of volunteer work. 2. The organizer has purchased third party insurance. If needed, participant can purchase his/her own personal or other related insurance. 3. The organizer suggests participant to consult doctor’s advice to make sure if his/her physical condition is suitable for the walk before registration and participation in the event. 4. The organizer deserves the right of final decision. Any changes or information with regard to the contingent measures for the event announced by the organizer and posted on the organizer’s Facebook page and official website shall prevail. 5. All awards will be based on the organizer’s official record. There will be no appealing mechanism, and the organizer reserves the right of final decision. 6.All the awards will only be presented at the designated time, and there will be no replacement, pre-collection or collection on behalf of the winners. 7. Upon submission of the registration form, it would mean that the participant acknowledges and agrees to accept all the race requirements, regulations and arrangements. 8. The organizer reserves the right of amending and explaining the above regulations, without prior notice.
